Countryside's Reopening Task Force has put together a brief survey to gather information about our readiness to return to in-person worship inside the church building. This survey should take approximately 5 minutes to complete. All responses will remain anonymous. You may choose to disclose your name or contact information if you would like to be contacted regarding your comments, concerns, or questions. This survey will close at 9:00 pm EDT on Friday, March 19, 2021
